A/a Gradient

The variables in this App. are :

RQ :Respiratory Quotient (approx 0.8 at normal physiological state)

PB : Atmospheric pressure.(760 mm Hg at sea level.)

FiO2 : Fraction of Inspired Oxygen. (0.21 at room air.)

PAO2 : Alveolar Oxygen tension

PaO2 : Arterial oxygen tension

These can be independently changed and the reflection of these changes can be seen in the alveolar – arteriolar gradient and PaO2 / FiO2 ratio.

A-a oxygen gradient : The alveolar arterial (A–a) oxygen gradient is a measure of oxygen transfer across alveolar capillary membrane (“A” denotes alveolar and “a” denotes arterial oxygenation). It is the difference between the alveolar and arterial oxygen tension.

A-a oxygen gradient = PAO2 - PaO2.

PaO2 is derived from ABG while PAO2 is calculated.

PAO2 = (FiO2 x [PB - PH2O]) - (PaCO2 ÷ RQ)

[PH2O is the partial pressure of water (47 mm Hg)] & PaCO2 is partial pressure of Carbon di oxide in arterial blood.

A-a gradient varies with age and can be estimated from the following equation, assuming the patient is breathing room air.

A-a gradient = 2.5 + 0.21 x age in years.

A-a gradient increases with higher FiO2.

PaO2/FiO2 ratio : It is a measure of Oxygen transfer across the alveolar capillary membrane. Normal PaO2/FiO2 ratio is 300 to 500 mmHg. Values less than 300 mmHg indicating impaired gas exchange and values less than 200 mmHg indicates severe hypoxemia.

ये स्वतंत्र रूप से बदला जा सकता है और इन परिवर्तनों का प्रतिबिंब वायुकोशीय में देखा जा सकता है - arteriolar ढाल और PaO2 / FiO2 अनुपात।

हूँ ऑक्सीजन ढाल: वायुकोशीय धमनी (ए-एक) ऑक्सीजन ढाल वायुकोशीय केशिका झिल्ली भर में ऑक्सीजन हस्तांतरण का एक उपाय ('ए' वायुकोशीय अर्थ और 'ए' धमनी ऑक्सीजन को दर्शाता है) है। यह वायुकोशीय और धमनी ऑक्सीजन तनाव के बीच का अंतर है।

एक-एक ऑक्सीजन ढाल = PAO2 - PaO2।

PAO2 गणना की जाती है, जबकि PaO2 एबीजी से ली गई है।

PAO2 = (FiO2 एक्स [पंजाब - PH2O]) - (PaCO2 ÷ RQ)

और PaCO2 धमनी रक्त में कार्बन di ऑक्साइड का आंशिक दबाव है [PH2O पानी का आंशिक दबाव (47 मिमी पारा) है]।

हूँ ढाल उम्र के साथ बदलता रहता है और मरीज के कमरे में हवा साँस ले रहा है, यह सोचते हैं निम्नलिखित समीकरण से अनुमान लगाया जा सकता है।

एक-एक ढाल = वर्षों में 2.5 + 0.21 एक्स उम्र।

उच्च FiO2 के साथ एक-एक ढाल बढ़ जाती है।

PaO2 / FiO2 अनुपात: यह वायुकोशीय केशिका झिल्ली भर में ऑक्सीजन हस्तांतरण का एक उपाय है। सामान्य PaO2 / FiO2 अनुपात 300 से 500 एमएमएचजी है। मूल्यों बिगड़ा गैस मुद्रा का संकेत कम से कम 300 एमएमएचजी और कम से कम 200 एमएमएचजी मूल्यों गंभीर हाइपोजेमिया इंगित करता है।
